Supported Living Services Waiver (SLS)

The SLS Waiver is built around individual choice — services and supports are tailored through a personalized planning process to address each person's prioritized, unmet needs. Rather than replacing the natural supports and community connections already in place, this waiver is designed to supplement them with targeted, cost-effective services that fill the gaps and help adults with I/DD thrive where they live.

Medicaid payment for services is available only after the Member is eligible for Medicaid Colorado Health First and the service is authorized. Not all individuals qualify. Timing of enrollment and service authorization may vary.

Provided on a short-term basis, because of the absence or need for relief of those persons who normally provide care for the participant. Respite may be provided in the participant's home, the caregivers home, or in the community. Respite is paid on an hourly basis with a cap of 10 hours within any 24 hour period.

The goal of mentorship is to promote self advocacy through methods such as instructing, providing experiences, modeling, and advising, including: Assistance in interviewing potential providers. Assistance in understanding complicated health and safety issues. Assistance with participation on private and public boards, advisory groups, and commissions.

Specialized and/or extraordinary cleaning tasks (cleaning, disinfection, and sanitization) to ensure a safe, hygienic living environment. When needing professional services, commercial grade products, and/or specialized cleaning services. Service must follow infection control protocols, handling and disposal of biohazard materials, and specialized materials or equipment.
